What is a questionnaire and why is it important?

A questionnaire is a module on a form, nearly always a wizard form, that is configured with a series of questions called the question set. A questionnaire collects information from individual external portal users. When the external user finishes answering the questionnaire, the application uses the information to create an answer set that internal application users can view.
